For the 2011 renewal season, the SALPN is offering an on-line process. Packages that were mailed in October contain a letter which indicates your User ID and Password along with instructions for completion. 

Each year renewal forms are sent to all practicing and non-practicing LPNs in October.  Completed renewal forms must be submitted by December 1.  Renewal forms submitted after the December 1 deadline are subject to a late fee.  Incomplete renewal forms are not accepted and are returned to the individual for completion, if the completed form is returned after the December 1 deadline, the late fee will apply.
